Allow seven (7) days to order scannable surveys from Learn and Serve
Arizona. Email your request to Kurt.Parks@azed.gov. For
consistency, all Arizona schools scannable surveys must be from one
printer for the state. Please, order an adequate quantify of survey
forms.
